MAT ANGELS - UNIVERSAL PARTICIPATION, RELEASE & CONSENT AGREEMENT

By submitting this form and selecting the acceptance checkbox below, I acknowledge and agree to the following terms on behalf of myself and/or the participant identified in this submission.

1. VOLUNTARY PARTICIPATION I understand that participation in Mat Angels programs, events, athlete support initiatives, fundraising activities, and related engagements is voluntary.

2. ASSUMPTION OF RISK I understand that participation involves inherent risks including injury, illness, or property damage and voluntarily assume all such risks.

3. RELEASE OF LIABILITY I release Mat Angels, its directors, officers, volunteers, partners, and affiliates from any liability arising from participation.

4. MEDICAL CONSENT I authorize necessary medical treatment in emergencies and accept responsibility for related expenses.

5. MEDIA RELEASE I grant rights to use photos, videos, and likeness for promotional purposes without compensation.

6. CODE OF CONDUCT I agree participant will act respectfully and understand removal may occur if standards are not met.

7. NO GUARANTEE OF SUPPORT I understand submission does not guarantee selection or funding.

8. FINANCIAL DISCLOSURE Donations are non-refundable unless otherwise stated.

9. DATA PRIVACY I consent to data use and communications including email and SMS.

10. MINOR CONSENT I certify authority if signing for a minor.
